Root Cause Analysis Project

In this project of 4 group mates, we had to research a major event that failed and then do a mini project/activity to explain in our own words why it failed. Then we presented our findings to the class and compared it to the actual results.

Elk River 

In 2014 a chemical company, Eastman Chemical Co., had a major crisis where their storage tanks that were holding a chemical called MCHM had failed and spilled gallons of harmful chemicals into the local's main water supply.

Research

In the pictures to the right are of me and my partners working on our poster that demonstrates our possible reasons of why the tanks leaked into the Elk River. Our final conclusion of why it leaked was because of poor maintenance & bad repair. After investigations officals found that the root cause of the issue was the those in charge of checking up on the tanks didn't realize that these tanks weren't up to code to hold a chemical as strong as MCHM. Therefore, the tank erroded and the chemicals spilled into the river, affecting hundreds of thousands who were without drinking water.
